The Medical Records department at Mark Reed Health Care District is here to serve your medical records needs in a professional, confidential manner. Whatever your needs may be – if you need to stop by and pick up copies of records or have them mailed for you to another provider, we are here to provide that service for you.
Mark Reed Healthcare Clinic has recently made the transition to an Electronic Medical Records (EMR) system in October. Many clinic records will be available in electronic format for more security and patient privacy and more efficient transmission of patient data between medical providers and pharmacies.
The Medical Records department is staffed from 8:00 a.m. to 4:30 p.m. Monday through Friday, but you may leave a message to request records at (360) 495-3244 ext. 252. To download our printable Authorization to Release Medical Records form, click here.

The Health Insurance Portability and Accountability Act (HIPAA) is a federal privacy rule that took effect April 14, 2003. It gives patients more control over their health information and greater protection of personal medical records. It also gives patients a choice of whether or not they want to be listed in the hospital's patient directory. Here are some changes you may encounter as a patient or a family member at hospitals:

- Patients will be given a Notice of Privacy Practices and asked to sign that they received it.
- Family members may be asked additional questions to confirm their relationship to a patient and their identity before information is released.
- Families and friends will only receive information if the patient has consented to this.
- Patients and families may be asked to designate a family communicator.
